Registration/Sign Up Forms

You've received your swim team registration approval email, Congratulations! 

Now what?

Below are the forms required to complete your swimmer's registration. These are mandatory for swim team membership. 

Registration fee: $75.00 per swimmer.

Forms 1 - 2 are electronic (see below to complete)

Form 3 must be printed out (2 for each swimmer), completed and given to our registrar - see note below for instructions.

Form 4 is for your information - there's no need to print (except for your files)

Form 5 of 5 must be printed out (1 for each swimmer), completed and given to our registrar - see note below for instructions.

    Kingfish Code of Conduct 2010/2011

    The code of conduct is an agreement between the swimmer and Kaiserslautern Kingfish Swim Team. Please remember that as a member of the Kingfish, you represent the team, your coaches, your parents and the entire Kaiserslautern Military Community. You should maintain a positive image and display the appropriate behavior at all times. I agree to the following rules:

    I. I will respect my body.
    a. Smoking, drinking alcoholic beverages and misusing drugs of any kind will not be tolerated and are grounds for
    immediate dismissal from the Kingfish Swim Team.
    b. I will respect the EFSL curfew of 2300 the night prior to a swim meet.
    c. I will respect pre-meet curfews set by the Kingfish coaching staff.
    II. I will respect the coaches
    a. I agree that decisions made by the coaches are final (Concerns must be brought to the team president)
    b. I will correct my behavior and attitude if found inappropriate.
    c. I will not enter the water until told to do so; and I will leave the water when told to do so.
    d. I understand that if I do not follow coaching staff directions I will be subject to disciplinary action.
    III. I will respect others and myself.
    a. I will not use crude or offensive language or gestures.
    b. I will honor the house rules set by my host family
    c. I will display good sportsmanship at all times.
    d. I will not intentionally interfere with the progress of another swimmer, physically or verbally.
    e. I will not take or use another swimmer’s property with out their permission.
    f. I will refrain from excessive public displays of affection at team practice, meets, bus or other events.
    IV. I will respect the team.
    a. I will support, assist and encourage my teammates at meets and at practice.
    b. I will support our team captains and help foster team spirit
    c. I will not wear clothing, swimsuits or caps that reflect negatively on the team—coach has final say.
    V. I will respect meet officials.
    a. I will abide by the rules f the host team.
    b. I will follow the directions of the Clerk of Course, Marshall and Meet Director.
    c. I will treat officials with respect and will abide by decisions of the Head Official.
    VI. I will respect pool rules.
    a. I will not participate in horseplay.
    b. I will not bring food or drinks on deck
    c. I will only enter or attempt to enter, the locker room appropriate for my gender.
    d. I will not vandalize or remove pool property.
    e. I will be courteous to the pool management.
    VII. I will respect the bus rules.
    a. I will remain seated at all times.
    b. I will not disturb others and will not display loud or obnoxious behavior.
    c. I will only use personal music devices with earphones on the bus
    d. I will pick up my trash before leaving the bus.
    e. I understand that chaperones have the authority to maintain discipline and order at all times.
    f. I know misbehavior may result in being barred from future bus trips for meets and practices.
    I fully understand that violations of this code may result in penalties that will be determined by the team Executive Board. Disciplinary actions may include suspension from practice and meets or dismissal from the team.

    I further agree to abide by the Code of Conduct sections II-VII.

    Kingfish Swim Team Financial Agreement - 2010/2011

    I understand and agree with the following obligations associated with membership in the Kaiserslautern Kingfish Swim Team.

    1. As a member of the Kaiserslautern Kingfish Swim Team, I agree to abide by the guidelines and rules established in the Team Handbook. I agree to follow the direction of the Meet Director or host team president while attending meets.

    2. Kingfish families are required to perform 60 hours of volunteer swim team service per family during the swim season. If volunteer hours are not fulfilled by the end of the season, the parents will be billed at a rate of $10.00/per uncompleted hour. Hours are tracked by the Kingfish Executive Board. Volunteer hour disputes should be brought to the attention of the team president. In addition, each family is required to work in the Concession Stand (“Bait Shop”) for one entire meet during the season (or 2 “half” meets) excepting those who hold a position required to run the meet (ex: Head Timer, Award Coordinator, Swim Shop Coordinator,
    Statistician, Clerk of Course, Marshall, and all officials). Only hours worked above this requirement will be included in the 60 hours total.

    3. Payment of an annual registration fee to be determined and approved by the Team Executive Board. The registration fee includes a team swim cap, team T-shirt, insurance, EFSL league dues, meet entry fees and a team handbook.

    4. Monthly dues will be determined and approved at the Team Executive Board. Dues are paid for the six months of the competitive season: September-February. Monthly dues should be paid to the team Treasurer at the monthly parents’ meeting or mailed to the address below by the 15th of each month.

    Note: it is possible to set up a repeating monthly payment through your bank’s web-check system.

    Swimmers may be suspended from practicing and competing if dues aren’t paid by the 16th of the month.  If U.S. Dollar to Euro exchange rate drops drastically, the team budget may be re-evaluated and monthly dues adjusted accordingly

    5. Official membership begins when the registration fee and the first month’s dues are paid and Financial Agreement is signed and returned.

    6. In order to participate in the league championship meet and to receive end of the season awards, all financial obligations must have been met.

    7. The season officially ends with the EFSL Championship Meet (CHAMPS) by may be continued, as per the constitution. All members in good standing with the team at the time of CHAMPS are considered members in good standing through the summer. Team memberships expire at the first parent’s meeting of the new swim season, at which time all members must join for the next season.
