Dear Kingfish Parents & Prospective Kingfish Parents,

Please note our team requirement:

Before the start of the first practice (in late August) all swimmers need a sports physical.

If your swimmer need a Sports Physicals go to your respective Clinic. Get there early!

Here are some FAQs and answers.

Question: Why does my child need to have a physical for swimming?

Answer: Competitive swimming is a physically demanding sport. We feel it is in the best interest of all swimmers to have a sports physical. In addition, our coaches need to be aware of any current medical conditions and or medications swimmers are on. Thus, we also require each swimmer to fill out a health history form.


Question: Do the coaches keep my child’s medical condition and or medications confidential?

Answer: Absolutely. All coaches will be informed that any information on any medical form is to be to be kept strictly confidential. If coaches have any questions, they can contact parents directly. 


Question: What forms do I use? Do I use the clinic forms or the Kingfish forms?

Answer: You'll need 2 forms.  For our records, we only need (1) the Sport and Youth Center Clearance form from the clinic and (2) the Emergency Medical Treatment Authorization and Trip Permission.


Question: My high school swimmer also plays high school soccer.  Is this form good for other high school sports as well?

Answer: Yes, although check with your respective high school's nurse to make sure.  If your swimmer intends to play a high school sport, make sure their school nurse gets a copy of the New Sport and Youth Center Clearance form.
